In traditional Chinese medicine, andrographis is used to remove heat, counteract toxicity and reduce swelling. In Ayurvedic and Thai medicine, it is used to treat fever associated with infectious diseases, and Scandinavians have used it as a cold and flu remedy.
The major active components in Andrographis paniculata extracts are diterpene lactones called andrographolides with several similar subcomponents such as neoandrographolides and dehydroandrographolides. Various beneficial actions have been identified for each subcomponent, as well as synergistic effects, including immune-modulating, antimicrobial, anti-inflammatory, cardiovascular, and anticancerous benefits.
Clinical Research and Use
In seven well-structured clinical trials andrographis has been shown to be superior to placebo in reducing subjective symptoms of upper respiratory tract infections (URTIs), and in one study was 52.7% more efficacious than placebo.
A 2017 meta-analysis investigated 33 randomized, controlled trials on andrographis treatment for acute respiratory tract infections (RTIs). The treatment group showed improved symptoms of cough and sore throat compared to placebo, and significant improvements in overall symptoms of RTIs when compared to placebo and conventional care. Additional outcomes included shortened duration of cough, sore throat and sick leave when compared to conventional care.1
Andrographolides have also been proposed as a treatment option for autoimmune disorders. In one double-blind, placebo-controlled clinical trial, patients with rheumatoid arthritis given andrographis for 14 weeks showed a significant decrease in the number and degree of swollen joints, and joint tenderness. Even more so, andrographis treatment was associated with a reduction in rheumatoid factor, IgA and C4.2
Mechanisms of Action
Andrographolides and neoandrographolides are thought to inhibit the growth of certain pathogens and inhibit access to cells by microbes and the toxins they produce. For example, andrographis is thought to inhibit intestinal secretions in diarrhea, which are induced by the toxins produced by the infectious bacteria.
Andrographis has also been compared to various medications for URTIs and colon infections and was found just as effective or better as conventional treatment.
