(R)-alpha-lipoic acid (RLA) is a naturally occurring fatty acid derivative that is involved in several key metabolic pathways including its role as an essential cofactor in mitochondrial energy production. RLA also regulates cellular signaling pathways over a timeline much greater than its own bioavailability. These effects include an increase in glutathione synthesis.
When administered exogenously, RLA provides several therapeutic benefits including neuroprotection from diabetic complications, reduction of age-associated oxidative damage, contributions to weight loss, and detoxification of redox-active transition metals.
RLA’s weight loss effects are attributed to its ability to help reduce food intake, increase energy expenditure, and in supporting blood sugar regulation. A meta-analysis of 11 placebo-controlled trials found that 8 to 52 weeks of RLA treatment is associated with significantly greater weight loss and BMI reductions compared to placebo.1
In a clinical trial of 181 diabetic patients, 5 weeks of RLA supplementation (600 mg daily) led to significant reduction in neuropathic symptoms from the baseline, compared to controls.2
Another trial assessed long-term use of RLA in 460 patients with mild to moderate diabetic neuropathy. After 4 years of treatment, 600 mg of RLA daily led to significant improvements in general neuropathic impairment, lower limb neuropathic impairment, and lower limb muscular weakness compared to controls for whom neuropathy progressed over the trial period.3
